Total Knee replacement

Knee arthritis is more common than hip arthritis.

The replacement is essentially replacing the worn our cartilage of the knee joint. The ligaments and joint capsule are retained, and the damaged cartilage and bone is simply resurfaced-using plastic on the end of the shin bone and metallic plate on the bottom of the thigh bone.

The artificial knee joint relieves pain, corrects the obvious deformity, and stops the joint from giving way.

Anesthetics

The anesthetic is one of the main concerns for patients, stemming from the fact that many feel they are handing over control of their life to another person. This worry is quite needless as modern anesthetics are very safe.

The anesthetist comes to the ward on the evening before the operation, asks questions concerning general health and any previous anesthetics, also examines the heart and chest, and will be pleased to answer any questions.

An hour or two before the operation an injection or tablets are given (the pre-med') to reduce anxiety, make the legs and lower part of the abdomen go numb.

This may be done either when the patient is awake or asleep. It is not too unpleasant, however nothing to worry about.

For the operation itself a full general anesthetic injection is given which either sends the patient completely to sleep or into a light sleep, with dim awareness of the surroundings from time to time. Patients may also, if they wish stay awake for the operation, but most prefer to be asleep.

Consciousness is regained fairly quickly afterwards but patients remain drowsy and may not remember much for several hours.

With an epidural anesthetic, patients find the numbness in their legs remains for an hour or two after the operation but they do not get much pain as the epidural is also used for pain relief. After a general anesthetic, patients are given pain-relieving injections for a day or so.
